Initial Comments:
This report is the result of special monitoring survey conducted on July 27, 2023, at LVHN Surgery Center which included closure of the facility located at 511 VNA Road, East Stroudsburg, PA 18301. Effective date of closure July 21, 2023. Based on the survey, it was determined the facility was in compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999.
